Please follow these detailed guidelines when completing your COC form to provide the best service possible.
Key Information to Include
✅ Company Name – Must match our records exactly, as this is how your report will be addressed. If your company name has changed or ownership has been transferred, please get in touch with QFSL to update our records before submission.
✅ Contact Person – Provide the name of the individual submitting the sample. This should be someone in Quality Assurance (QA) or the relevant department who can assist with any inquiries related to the sample.
✅ Phone Number & Email Address – These details are critical in case we need to contact you regarding:
- A presumptive positive result
- Next steps for testing
- Temperature deviations upon sample arrival
- Weather delays or other urgent updates
We primarily communicate via email, so please check regularly throughout the testing process to avoid delays.
Project Information
This section allows you to provide a clear description of your testing request. Please specify:
- Test Type – e.g., weekly E. coli testing, shelf-life study, or specific product testing
- Lot or Batch Identification – Unique names or codes for tracking
Providing this information helps us accurately log your sample and makes it easier for you to retrieve historical reports when needed. Without these details, searching through hundreds of reports may take longer.
Report & Submission Details
- Relinquished By – The person authorizing QFSL to proceed with testing.
- Date & Time Sent – When the sample was shipped.
- Shipping Carrier – Indicate whether you used FedEx, UPS, USPS, or another provider.
This ensures a smooth and traceable process from sample submission to results reporting.
Requested Services
- Sample ID – Assign a unique identifier to each sample (e.g., 1, 2, 3 or A, B, C).
- Sample Description – Include relevant details such as lot numbers, product type, composite sample info, or special reporting requests.
Once you’ve assigned sample IDs and descriptions, select the required tests by checking the appropriate boxes. If you need multiple tests that are not listed, please:
- Contact the QFSL team for assistance.
- Use a Client Editable COC form for custom requests.

Once your COC is completed and you are ready to ship your sample:
